All patients participating in the Prostate Health Clinic are required to sign the following disclaimer.
Please take time to read and understand the information below to confirmd that you understand the nature, benefit, limitations and risks of the service provided by the Prostate Health Clinic.
1. Purpose of the Assessment
The Prostate Health Clinic provides a comprehensive men’s health evaluation of the five pillars of prostate health, including prostate cancer risk assessment, urinary tract symptom assessment, erectile function assessment, testosterone and hormonal evaluation, cardiovascular health screening, and personalised recommendations.
This service is a screening and advisory programme, not a diagnostic medical service or a substitute for ongoing care by your GP or specialist.
The evaluation offered by the Prostate Health Clinic includes the following assessment as part of the package:
Clinical evaluation
Two consultant urologist clinic appointments combining clinical history and focused physical examination
Validated clinical questionnaires
Height, weight and blood pressure assessment
Urine dipstick analysis X2
Urinary flow rate and post void bladder urine residual ultrasound
Blood tests
Full blood count
Urea and Electrolytes
Liver Function tests
Bone profile
Magnesium
Testosterone
Lutenising Hormone and Follicular Stimulating Hormone
Cholesterol
HbA1c
Bi-parametric prostate MRI scan
10 year Cardiovascular risk assessment
Health assessment report and recommendation
2. Limitations of Screening & Risk Assessment
No screening test can detect all cases of disease.
A normal or low‑risk assessment does not guarantee you are free from prostate cancer or any other conditions at the time of assessment or in the future
Results reflect your health status only at the time of testing.
You may still develop prostate cancer or other conditions in the future even if considered low‑risk.
The health conditions assessed may improve with recommended lifestyle changes and medical treatments. However, even under optimal circumstances, aspects of the prostate health pillars may deteriorate over time and may require re-assessment and additional treatment recommendations
3. Nature of Advice Provided
The clinic provides general medical advice and interpretation of screening results. The service does not replace the role of your GP. You should seek urgent medical attention if new or worsening symptoms develop.
Lifestyle recommendations provided by The Prostate Health Clinic are intended to support improvements in general and prostate health. These recommendations require self-directed implementation, and any potential benefits are dependent upon the individual’s adherence to the suggested modifications. The Clinic cannot guarantee specific outcomes, as results may vary based on the level of compliance and other personal factors.
4. Responsibility for Providing Accurate Information
Confirmation of booking into a specialist urologist clinic for the advertised Prostate Health Clinic assessment will only be confirmed upon upfront receipt of the full amount payable
You confirm that all information you provide is accurate and complete. Failure to disclose relevant information may affect interpretation of your results.
The Prostate Health Clinic is not designed to monitor individuals with a prior diagnosis of prostate cancer
The safe conduct of the bi-parametric prostate MRI scan requires disclosure of the following MRI contraindications through the enquiry systems:
Severe Claustrophobia
Pacemaker
Intracranial aneurysmal clip
Implanted metal
Failure to disclose accurate MRI safety information will result in the MRI component of the prostate assessment being cancelled. It is the patient’s responsibility to declare all relevant information. Omission or misrepresentation of MRI safety details will render the patient ineligible for any refund related to the MRI portion of the assessment and limited the outcome and quality of the Prostate Health Clinic assessment report.
The Prostate Health Clinic assessment requires completion of the full portfolio of evaluations outlined in Section 1. Any decision by the patient to omit components of this assessment is entirely at their discretion; however, partial completion will render the patient ineligible for refund.
5. Follow‑Up and Ongoing Care
The Prostate Health Clinic assessment includes two clinic attendances.
You understand that follow‑up appointments or further testing may be advised. It is the patients responsibility to arrange these. Onward referrals should be made via the patients GP unless otherwise stated.
The prostate health clinic can support / signpost patients to allied health specialists outside the NHS, which will be subject to additional costs.
The Prostate Health Clinic may recommend medical therapies to support the management of prostate or cardiovascular health concerns. Ongoing monitoring or follow-up of any recommended treatment is not included within the Prostate Health Clinic assessment package. Patients should discuss any required quotations for continued care with their assessing urologist following completion of the assessment. Alternatively, patients may seek ongoing management through their NHS general practitioner or via referral to local NHS urology services.
The Prostate Health Clinic does not provide a surgical intervention service for prostate-related health concerns. Should surgical management be recommended, patients may obtain separate quotations through their assessing urologist. Alternatively, appropriate onward referrals can be arranged via the patient’s NHS general practitioner
Additional prostate cancer biomarkers are offered through the Prostate Health Clinic. These fall outside the advertised price of this comprehensive prostate health assessment. Please discuss quotations and indications for these with your assessing urologist.
The Prostate Health Clinic may identify a risk of prostate cancer. Recommendations for further investigation with a prostate biopsy may be advised to evaluate this further. A prostate biopsy procedure is not included as part of the Prostate Health Clinic assessment package. This procedure can be conducted through the expert urology team working at the hale private clinic. Please discuss quotations with your assessing urologist. Alternatively a urgent suspected cancer referral recommendation from your GP to your local NHS urology team will be recommended.
6. Data Protection and Confidentiality
Your information will be handled according to UK GDPR and our Privacy Policy.
Your information will be eld securely using the electronic patient record system “Carebit.”
7. Fees and payment
The assessment fee covers only the services listed in section 1. Any additional tests or referrals may incur extra charges.
A late cancellation fee of £250 will be payable within 48 hour’s notice of cancellation. The remaining balance will be re-funded to your account. Cancellations over 48 hours from the will not result in a cancellation fee.
Cancellation and rebooking will not result in a cancellation fee.
8. Consent to undergo the comprehensive prostate health assessment
By signing this document, you agree that:
You understand the purpose and limitations of the assessment.
You acknowledge that screening cannot guarantee prevention or detection of all disease.
You consent to undergo the tests included in your assessment package.
9. Acknowledgement & Acceptance
